How Much Bac Water for 20mg Retatrutide in Units
How much bac water for 20mg retatrutide comes down to 1, 2 or 3 mL. On 22 October 2026, 3 of the top 10 US results for this search gave 2 mL as their main answer for a 20 mg vial; one listed 1 or 2 mL, and a Reddit thread said 1 mL. That gives 10 mg in every mL, so 1 mg is 10 units on a U-100 insulin syringe.
Short answer for a 20 mg vial
- 1 mL of bac water: 20 mg per mL, and 1 mg is 5 units.
- 2 mL of bac water: 10 mg per mL, and 1 mg is 10 units.
- 3 mL of bac water: 6.67 mg per mL, and 1 mg is 15 units.
Disclaimer: this converts milligrams into syringe units. It is not a dose for you, and only a licensed clinician should set one.
This page turns every weekly dose from the published retatrutide trials into syringe units. None of the 6 top results we read in full that day showed all three water volumes for every vial size. Here you get all three water volumes for six vial sizes, with the draws that will not work marked.

How many units is each trial dose in a 20mg vial?
The table below shows the units for every weekly dose used in the main retatrutide trials. First, here is where those doses come from.
The published retatrutide trials gave these once-weekly doses:
Phase 2 obesity trial (2023): 1, 4, 8 or 12 mg, with the higher doses started at 2 or 4 mg
Phase 2 type 2 diabetes trial (2023): 0.5, 4, 8 or 12 mg
TRIUMPH-1 (2026): 4, 9 or 12 mg, started at 2 mg and raised every 4 weeks, passing 6 mg on the way to 9 and 12 mg
Disclaimer: these once-weekly doses were given to people inside monitored clinical trials. They appear here only to show the math and are not advice to take any amount. Retatrutide has no approved dose, so that choice sits with a licensed clinician.
| Weekly trial dose | 1 mL (20 mg/mL) | 2 mL (10 mg/mL) | 3 mL (6.67 mg/mL) | Full draws per vial |
|---|---|---|---|---|
| 0.5 mg | 2.5 (small draw) | 5 | 7.5 | 40 |
| 1 mg | 5 | 10 | 15 | 20 |
| 2 mg | 10 | 20 | 30 | 10 |
| 4 mg | 20 | 40 | 60 | 5 |
| 6 mg | 30 | 60 | 90 | 3 |
| 8 mg | 40 | 80 | 120 (over 1 syringe) | 2 |
| 9 mg | 45 | 90 | 135 (over 1 syringe) | 2 |
| 12 mg | 60 | 120 (over 1 syringe) | 180 (over 1 syringe) | 1 |
How to read it: "over 1 syringe" means the draw is more than 100 units, the most a 1 mL syringe holds. "Small draw" means under 5 units, which is hard to measure. Full draws per vial is the 20 mg divided by the dose, rounded down. It is the same at any water volume.

How much bac water for 10mg, 30mg, 40mg, 15mg and 5mg retatrutide?
The same rule works for every vial. The bigger the vial, the stronger each mL, and the fewer units each dose takes. These tables use the same trial doses as above.
How much bac water for 10mg retatrutide?
With 2 mL, a 10 mg vial holds 5 mg per mL, so 1 mg is 20 units. With 1 mL it is 10 units. This vial is too small for the 12 mg trial dose.
| Weekly trial dose | 1 mL (10 mg/mL) | 2 mL (5 mg/mL) | 3 mL (3.33 mg/mL) | Full draws per vial |
|---|---|---|---|---|
| 0.5 mg | 5 | 10 | 15 | 20 |
| 1 mg | 10 | 20 | 30 | 10 |
| 2 mg | 20 | 40 | 60 | 5 |
| 4 mg | 40 | 80 | 120 (over 1 syringe) | 2 |
| 6 mg | 60 | 120 (over 1 syringe) | 180 (over 1 syringe) | 1 |
| 8 mg | 80 | 160 (over 1 syringe) | 240 (over 1 syringe) | 1 |
| 9 mg | 90 | 180 (over 1 syringe) | 270 (over 1 syringe) | 1 |
| 12 mg | more than the vial holds | more than the vial holds | more than the vial holds | none |
How much bac water for 30mg retatrutide?
With 3 mL, a 30 mg vial gives 10 mg per mL and puts 1 mg at 10 units. At 1 mL, the smallest trial doses fall under 5 units.
| Weekly trial dose | 1 mL (30 mg/mL) | 2 mL (15 mg/mL) | 3 mL (10 mg/mL) | Full draws per vial |
|---|---|---|---|---|
| 0.5 mg | 1.7 (small draw) | 3.3 (small draw) | 5 | 60 |
| 1 mg | 3.3 (small draw) | 6.7 | 10 | 30 |
| 2 mg | 6.7 | 13.3 | 20 | 15 |
| 4 mg | 13.3 | 26.7 | 40 | 7 |
| 6 mg | 20 | 40 | 60 | 5 |
| 8 mg | 26.7 | 53.3 | 80 | 3 |
| 9 mg | 30 | 60 | 90 | 3 |
| 12 mg | 40 | 80 | 120 (over 1 syringe) | 2 |
How much bac water for 40mg retatrutide?
A 40 mg vial is strong at any of these volumes. Even with 3 mL (13.33 mg per mL), the 0.5 mg trial dose is only 3.8 units. Every trial dose fits one 1 mL syringe at 2 or 3 mL.
| Weekly trial dose | 1 mL (40 mg/mL) | 2 mL (20 mg/mL) | 3 mL (13.33 mg/mL) | Full draws per vial |
|---|---|---|---|---|
| 0.5 mg | 1.3 (small draw) | 2.5 (small draw) | 3.8 (small draw) | 80 |
| 1 mg | 2.5 (small draw) | 5 | 7.5 | 40 |
| 2 mg | 5 | 10 | 15 | 20 |
| 4 mg | 10 | 20 | 30 | 10 |
| 6 mg | 15 | 30 | 45 | 6 |
| 8 mg | 20 | 40 | 60 | 5 |
| 9 mg | 22.5 | 45 | 67.5 | 4 |
| 12 mg | 30 | 60 | 90 | 3 |
How much bac water for 15mg retatrutide?
With 2 mL, a 15 mg vial holds 7.5 mg per mL. 1 mg is 13.3 units, and the 8 mg and larger trial doses no longer fit one 1 mL syringe.
| Weekly trial dose | 1 mL (15 mg/mL) | 2 mL (7.5 mg/mL) | 3 mL (5 mg/mL) | Full draws per vial |
|---|---|---|---|---|
| 0.5 mg | 3.3 (small draw) | 6.7 | 10 | 30 |
| 1 mg | 6.7 | 13.3 | 20 | 15 |
| 2 mg | 13.3 | 26.7 | 40 | 7 |
| 4 mg | 26.7 | 53.3 | 80 | 3 |
| 6 mg | 40 | 80 | 120 (over 1 syringe) | 2 |
| 8 mg | 53.3 | 106.7 (over 1 syringe) | 160 (over 1 syringe) | 1 |
| 9 mg | 60 | 120 (over 1 syringe) | 180 (over 1 syringe) | 1 |
| 12 mg | 80 | 160 (over 1 syringe) | 240 (over 1 syringe) | 1 |
How much bac water for 5mg retatrutide?
A 5 mg vial holds less than the 6, 8, 9 and 12 mg trial doses, so those rows are empty. With 2 mL it is 2.5 mg per mL, and 1 mg is 40 units.
| Weekly trial dose | 1 mL (5 mg/mL) | 2 mL (2.5 mg/mL) | 3 mL (1.67 mg/mL) | Full draws per vial |
|---|---|---|---|---|
| 0.5 mg | 10 | 20 | 30 | 10 |

How do you check the math yourself?
You only need three steps and a calculator. Here they are for a 20 mg vial mixed with 3 mL, using the 2 mg starting step from the trials.
- Strength: 20 mg divided by 3 mL is 6.67 mg per mL.
- Volume: 2 mg divided by 6.67 mg per mL is 0.3 mL.
- Units: 0.3 mL times 100 is 30 units.
Now run it backwards. 30 units is 0.3 mL, and 0.3 mL times 6.67 mg per mL is 2 mg. If you get back the number you started with, the math is right.
How many mg are in 10 or 20 units?
Divide the units by 100 to get mL, then multiply by the mg per mL. For a 20 mg vial:
| Bac water | 10 units holds | 20 units holds |
|---|---|---|
| 1 mL | 2 mg | 4 mg |
| 2 mL | 1 mg | 2 mg |
| 3 mL | 0.67 mg | 1.33 mg |
For any other vial or amount, the retatrutide reconstitution calculator runs the same three steps as you type.

Should you use 1, 2 or 3 mL of bac water?
No volume is right for everyone. Each one trades big doses against small ones. The units in the 20 mg table show the trade.
- 1 mL keeps large amounts small. The 12 mg trial dose is 60 units. But 0.5 mg shrinks to 2.5 units.
- 2 mL sits in the middle. 1 mg is 10 units, so the math is easy. The 12 mg trial dose, at 120 units, needs two draws.
- 3 mL spreads small amounts out. 0.5 mg becomes 7.5 units. But 8 mg and up will not fit in one 1 mL syringe.
Small draws matter more than they look. In a 2004 bench test, insulin syringes were badly off at 1 unit and less accurate at 2 units than pens or a pump. Syringes with half-unit marks did not fix it.
Syringe size matters too. The FDA device records list 0.3, 0.5 and 1 mL insulin barrels, holding 30, 50 and 100 units. Our insulin syringe units chart shows how each mark maps to mL.

How do you mix bac water into a 20mg vial?
No label exists for retatrutide. What follows is a report of what the labels of approved powder injections, and of bac water itself, tell their own users.
- The Novarel label says to wipe the vial tops with an alcohol pad and not to touch them afterwards.
- The Hospira bac water label calls for germ-free handling whenever a vial is entered.
- The Serostim label says to aim the stream of water against the glass wall of the vial, which stops it foaming.
- The EGRIFTA WR label says to swirl the vial in a circle and not to shake it. The Serostim label says the same.
- The Serostim and EGRIFTA WR labels say not to use a mix that is cloudy or has particles. The bac water label also says to look for any color change.
- CDC guidance asks clinics to put the opening date on any multi-dose vial.
None of these labels says how much water a retatrutide vial needs; the tables above only show the math for 1, 2 and 3 mL. For more on each step, see how to use peptides.

How long does mixed retatrutide last?
No official source says. With no approved label, nobody has published an in-use time for a mixed retatrutide vial.
- The Hospira bac water label stores the water itself at 20 to 25 C. It also warns against keeping any mixed drug unless that drug's own maker allows it.
- CDC guidance for clinics gives an opened multi-dose vial 28 days before it is thrown out, unless its maker sets a different date. That rule covers vials in general, not any one peptide.
- All 6 top results we read in full on 22 October 2026 say to keep a mixed vial in the fridge. That advice comes from those guides, not from a retatrutide label.
What the labels say about the water before and after opening is covered in does bacteriostatic water need to be refrigerated. For heat and travel, see storing peptides in Vietnam heat.

Why use bac water for retatrutide?
A 20 mg vial is drawn from many times, so the water needs a preservative. Bac water has benzyl alcohol in it, and the label sells it as a multiple-dose vial. Plain sterile water has no preservative.
Our guide to what bac water is reads a real label line by line, and bac water vs sterile water covers the difference in full.

Common questions
How much bac water for 20mg retatrutide in units?+
The water you add sets the units. With 2 mL, a 20 mg vial holds 10 mg per mL, so 1 mg is 10 units on a U-100 insulin syringe. With 1 mL, 1 mg is 5 units. With 3 mL, 1 mg is 15 units. The total in the vial stays 20 mg either way. Disclaimer: this converts milligrams into syringe units. It is not a dose for you, and only a licensed clinician should set one.
How much bac water to reconstitute 10mg retatrutide?+
With 2 mL, a 10 mg vial holds 5 mg per mL, so 1 mg is 20 units. With 1 mL it is 10 mg per mL and 1 mg is 10 units. Disclaimer: this converts milligrams into syringe units. It is not a dose for you, and only a licensed clinician should set one.
What does 3 mL of bac water do to a 20mg retatrutide vial?+
It makes 6.67 mg per mL. Each 1 mg then takes 15 units. Of the weekly doses in the published trials, 8, 9 and 12 mg would each need more than one full 1 mL syringe at this mix. Disclaimer: these once-weekly doses were given to people inside monitored clinical trials. They appear here only to show the math and are not advice to take any amount. Retatrutide has no approved dose, so that choice sits with a licensed clinician.
How many mg is 20 units of retatrutide?+
It depends on the mix. In a 20 mg vial, 20 units holds 4 mg with 1 mL of water, 2 mg with 2 mL, and 1.33 mg with 3 mL. To work it out, divide the units by 100, then multiply by the mg per mL. Disclaimer: this converts milligrams into syringe units. It is not a dose for you, and only a licensed clinician should set one.
Does more bac water make retatrutide weaker?+
No. The water changes how much liquid holds each milligram, not how many milligrams are in the vial. A 20 mg vial holds 20 mg whether you add 1, 2 or 3 mL. Only the number of units for a given amount changes.
Is retatrutide approved?+
No. Retatrutide is still an investigational drug with no approved label anywhere. Its maker, Eli Lilly, has said it plans to file for US approval in the first quarter of 2027. Every dose on this page comes from a clinical trial.
