Cagrilintide Dosage Chart and Calculator: Units by Vial

Cagrilintide dosage chart numbers come from three human trials, all funded by Novo Nordisk, the company developing it. Each one gave a shot under the skin once a week. We found no US FDA approval for it in openFDA, so there is no label dose to quote. This page lists the trial amounts and turns them into syringe units for 5 mg and 10 mg vials.

Short answer

Three Lancet trials gave cagrilintide as a weekly shot, and no reader should treat those amounts as a personal dose. Each amount sits in the chart below next to its paper. The units table then turns those amounts into syringe marks for 5 mg and 10 mg vials.

For the evidence on the molecule itself, read the cagrilintide profile. This page handles the numbers.

Cagrilintide calculator

Type in your vial mg, your water volume and a figure in mg. You get the strength, the units on a U-100 syringe, and how many times that amount fits in the vial. The starting values come from the tables below.

Which cagrilintide trials give a human dose?

Our profile page cites four papers. We opened each abstract on 23 October 2026. Three of them are human trials with stated amounts. The fourth, a 2015 review of amylin by Hay and colleagues, gives no cagrilintide dose at all.

The three trials, as published in The Lancet in 2021 and 2023, gave these weekly cagrilintide amounts:

The amounts were not started at the top. The phase 2 trial allowed up to 6 weeks of step-ups. The phase 1b trial raised the amount every 4 weeks over 16 weeks, then held it for 4 more. The abstracts do not print every step, so this page does not invent them.

Is cagrilintide FDA approved? What we checked

No US FDA approval for cagrilintide turned up when we searched openFDA on 23 October 2026. Its drug approvals search had no match for cagrilintide or for CagriSema. On 18 December 2025, Novo Nordisk said it had filed a US new drug application for CagriSema, which is cagrilintide given with semaglutide.

In that release, Novo Nordisk calls CagriSema an investigational medicine. The FDA also states that cagrilintide cannot be used in compounding under US federal law. We did not check drug agencies outside the US, so this covers only these US records. Our CagriSema page covers the combination trials.

Cagrilintide units for 5 mg and 10 mg vials

Method: strength is vial mg divided by water mL. Units are the trial amount divided by strength, times 100. A script built every cell, rounded to one decimal place. Where an amount needs more than 1 mL, the cell shows the volume instead.

Taking the weekly amounts from the Lau phase 2 trial in The Lancet, our script gives these syringe units:

U-100 units for each phase 2 trial amount, by vial and water volume.

Disclaimer: these cells are arithmetic that turns the trial amounts into syringe units. They plan nothing for anyone; dosing calls stay with a licensed clinician.

Two patterns show up. A 10 mg vial with 2 mL reads exactly like a 5 mg vial with 1 mL, because both come to 5 mg per mL. And at 3 mL in a 5 mg vial, the two highest amounts no longer fit in one 1 mL syringe. For reading the marks, see the syringe units chart.

How many trial amounts fit in one vial?

This is vial mg divided by each amount. Water does not change it. We left the result as a decimal, so a partial amount at the end of a vial shows clearly.

Dividing each Lau phase 2 amount from The Lancet into the vial gives these counts:

How many times each phase 2 amount fits in a vial. Script output, one decimal place.
Vial0.3 mg0.6 mg1.2 mg2.4 mg4.5 mg
5 mg16.78.34.22.11.1
10 mg33.316.78.34.22.2

Disclaimer: each cell counts how many times one trial amount fits in a vial. It is vial arithmetic, not a plan or a dose, and dosing calls stay with a licensed clinician.

The trials gave one amount a week, so a count here also reads as a count of weeks. This page sets no shelf life for a mixed vial, since none of these sources gives one. Our guide to storing peptides in Vietnam heat covers handling.

What we checked on 23 October 2026

The vial sizes in the table came from somewhere, so we checked them. On 23 October 2026 we opened six public seller listings for cagrilintide. Four loaded with readable text and two blocked us. This is our own snapshot, not a survey.

  • 4 of 4 sold a 5 mg vial.
  • 3 of 4 also sold a 10 mg vial.
  • 1 of 4 printed a water volume, 3 mL for the 5 mg vial.
  • 1 of 4 printed the size of the vial glass, also 3 mL.

That 3 mL glass comes from one listing only, so it is not a water limit for every vial. It is the reason the table stops at 3 mL. We also typed every unit cell into the calculator above. All 30 cells matched the table.

Common questions

How many units of cagrilintide are in 1 mg?+

That depends on the water you mixed in. Divide the vial mg by the water mL to get the strength, then divide 100 by that strength. The units table below works this through for 5 mg and 10 mg vials at three water volumes.

What size vials does cagrilintide come in?+

In the four seller listings we could read on 23 October 2026, all four sold a 5 mg vial and three also sold a 10 mg vial. That is our own small snapshot. Always go by the mg printed on the label you hold.

How much bac water goes in a 5 mg cagrilintide vial?+

No trial or approved label sets a volume of bacteriostatic (bac) water. The water only changes the strength, not the mg in the vial. Mind the glass, too. One listing we read gave a 3 mL vial, and that is one listing only. Check the size of your own vial before you add water.

Is cagrilintide approved by the FDA?+

We found no US FDA approval for it. The openFDA approvals search had no match on 23 October 2026. Novo Nordisk filed in the US on 18 December 2025 for CagriSema, the pairing with semaglutide, and calls CagriSema investigational.

Can you mix cagrilintide with another drug in one vial?+

The abstracts we read say the drugs were given together each week. They say nothing about mixing two drugs in one vial yourself. This calculator handles one compound in one vial, so it cannot work out a blend.
