What Tretinoin Is

Tretinoin is all-trans retinoic acid, the active acid form of vitamin A. In the wider retinoid family, most ingredients have to be converted inside the skin before they can do anything. Tretinoin is already the finished, active molecule, which is why it is a prescription drug rather than a cosmetic in most places. The chemistry is settled: a single compound, molecular formula C20H28O2, catalogued in PubChem as tretinoin, retinoic acid, and vitamin A acid.

That one detail explains most of what follows. Because tretinoin is the active acid itself, it acts faster and harder than the over-the-counter forms that need converting, and it is also more likely to irritate. Where a product sits on that trade-off, from a gentle cosmetic to prescription tretinoin, is the whole reason the retinoid aisle is so confusing.

Topical Cream vs Oral Capsule: Two Different Drugs

This is the distinction that trips people up, so it goes first. Tretinoin is one molecule, but it reaches the market as two completely different medicines that share nothing except the active ingredient.

Topical tretinoin

A cream or gel applied to the skin. This is what almost everyone means by tretinoin. Its US labels cover acne, and one emollient cream is approved for fine facial wrinkles. Everything on this page, unless it says otherwise, is about this form.

Oral tretinoin

A capsule of the same molecule, and an oncology drug. US labeling indicates it for inducing remission in acute promyelocytic leukaemia, a blood cancer, in patients whose disease has relapsed or has not responded to anthracycline chemotherapy, or who cannot receive it.

The approval, the dose, and the risks of the capsule have nothing to do with the skin cream, and none of the cosmetic use of the cream transfers to the capsule. Whenever you read a claim about tretinoin, the first question is which one is being talked about. Every label fact on this page comes from a topical tretinoin label, unless it is explicitly about the oral capsule.

How It Works

Topical tretinoin activates the three retinoic acid receptors found in skin cells, known as RAR alpha, RAR beta, and RAR gamma. Switching those receptors on can change how genes are expressed and how skin cells grow and mature. That is the theory, and the labels are careful about how far to push it.

The US prescribing information states plainly that it has not been established whether the clinical effects of tretinoin are actually mediated through those receptors or through other mechanisms, and that the exact mechanism of action in acne is unknown. That is unusually honest wording for a drug this well used, and it is worth carrying both halves: the receptor activity is real, the causal link to results is not fully proven.

What the evidence does point to, in the acne label, is more concrete. Tretinoin makes the cells lining the pore less sticky, so fewer microcomedones, the tiny starting clogs, form in the first place. It also stimulates the turnover of those cells, which helps push existing plugs out. Speeding up cell turnover is also the mechanism behind the tingling, flaking, and dryness that come with it, which is why benefit and irritation tend to arrive together.

What The Labels Approve It For

Topical tretinoin has been prescription medicine in the US for decades, and the approved uses are narrower than the reputation suggests. Two things are on-label.

Acne vulgaris

This is the core approval. The classic label states that tretinoin is indicated for topical application in the treatment of acne vulgaris, and adds that the safety and efficacy of long-term use in other disorders have not been established. The newer microsphere gel carries the same acne indication for adults and children 12 years of age and older.

Fine facial wrinkles, in one specific product

There is a separate, narrower approval for aging skin. One emollient tretinoin cream, RENOVA 0.02%, is approved in the US as an adjunctive agent for the mitigation of fine facial wrinkles, in people who are already using a comprehensive sun-protection and skin-care program. The same label is unusually blunt about the ceiling: it states in capital letters that the product does not eliminate wrinkles, repair sun-damaged skin, reverse photoaging, or restore younger skin, and that the 0.02% strength did not demonstrate an effect on coarse or deep wrinkling, tactile roughness, or mottled hyperpigmentation. People with visible actinic keratosis or a history of skin cancer were excluded from those trials, so the result does not extend to them.

Everything else you hear tretinoin credited with, from big anti-aging transformations to fading every kind of dark spot, sits outside those two label lines. Some of it has research behind it and some does not, but it is off-label, and that is a useful line to keep in mind when a seller makes a promise.

What The Human Evidence Shows

The aging-skin story rests on real, if early, human data. The study people still point to was published in JAMA in 1988 by Weiss and colleagues at the University of Michigan: a 16-week randomized, double-blind, vehicle-controlled trial of topical tretinoin for photoaging.

  • Thirty people completed the study. Each applied tretinoin to one forearm and a plain vehicle cream to the other, and half of the group also received tretinoin on the face while the other half received the vehicle on the face.
  • Every completer showed a statistically significant improvement in photoaging on the tretinoin forearm, but not on the vehicle forearm.
  • 14 of the 15 people who used tretinoin on the face improved, while none of the vehicle-treated faces did, a significant difference between the two groups.
  • Skin biopsies showed matching structural changes in the tretinoin-treated skin. Side effects were limited to irritation of the treated skin.

It is a small, early trial in a US population, not proof of a miracle. What it does is put tretinoin in a category most cosmetic actives never reach: a controlled, blinded, vehicle-compared human study where the treated skin measurably changed and the untreated skin did not. That is the difference between an ingredient with evidence and an ingredient with marketing.

Who It Is For

Two groups have a label-backed reason to consider topical tretinoin: people with acne, which is the core approved use, and people bothered by fine facial lines, where the RENOVA-style approval sets a realistic and deliberately limited expectation. Because it is a prescription medicine in the US, whether it fits, and at what strength, is a clinician decision rather than a self-serve one.

A few things hold across the topical labels and are worth knowing before anyone starts:

  • Tretinoin makes skin more sensitive to sunlight, so daily sun protection is part of the picture, and it is used at night.
  • Irritation, dryness, peeling, and redness are the common listed reactions, and they tend to be worst early on.
  • Retinoids as a class are generally avoided in pregnancy, which is a conversation for a doctor, not a guide.
