Wegovy Diarrhea: How Common It Is and Why It Happens

Wegovy diarrhea is common. In the trials behind the FDA label for Wegovy, 30% of adults on Wegovy injection had diarrhea, against 16% on placebo. A 2022 tolerability study found 98.1% of gut side effects on semaglutide were mild to moderate. They came most often in the weeks when the dose went up, and they tended to fade with time.

Short answer

How common is it?
About 1 in 3 people on Wegovy injection in the label trials, against about 1 in 6 on placebo.
When does it happen?
Mostly while the dose is being raised, or soon after a raise.
What does the label warn about?
Fluid loss. Diarrhea can lead to dehydration, and dehydration may cause kidney problems.

Will Wegovy Cause Diarrhea?

It often does. In the Wegovy label's trials, 30% of people on Wegovy injection reported diarrhea, against 16% on placebo.

The Wegovy label lists diarrhea among its most common reactions. In that label, only nausea was reported more often. Wegovy is semaglutide, a GLP-1 receptor agonist.

A 2026 expert review calls nausea, vomiting, diarrhea and constipation class effects of these drugs. The review says they occur in about 30% to 50% of patients. It says they come mostly when the drug is started or the dose is raised.

The STEP 1 trial, published in 2021, found the same pattern. In STEP 1, nausea and diarrhea were the most common side effects of semaglutide. For the wider list of side effects, see our GLP-1 side effects guide.

How Common Is Wegovy Diarrhea?

In the Wegovy label's trial table, 30% of people on Wegovy had diarrhea. On placebo, 16% did.

So it happened in nearly twice as many people on the drug. The label table covers 2,116 people on Wegovy injection. It covers 1,261 people on placebo. The dose in that table is shown below, as the label prints it.

The Wegovy label's side effect table lists the dose studied as:

Wegovy injection 2.4 mg once weekly

A 2022 tolerability study looked at stomach side effects with semaglutide 2.4 mg. In that study, 29.7% of people on the drug had diarrhea, against 15.9% on placebo. The same study found nausea in 43.9% on the drug and 16.1% on placebo. These numbers line up closely with the label.

Younger patients were studied too. In the Wegovy label's pediatric trial, 22% on Wegovy had diarrhea and 19% on placebo did. In that pediatric trial, 62% on Wegovy had some stomach or gut reaction, against 42% on placebo.

All rows are from the Wegovy prescribing information. The pediatric rows come from its pediatric weight-loss trial.

The placebo column matters. In the Wegovy label's trials, 16% of adults on placebo still had diarrhea, so not every case on Wegovy came from Wegovy. The gap between the two columns is the part linked to the drug. That gap is much smaller in the pediatric trial, at 22% and 19%.

Why Does Wegovy Cause Diarrhea?

The sources link it to dose increases. They do not explain the exact body process behind it.

Wegovy is started at a low dose and raised in steps. The Wegovy label says to follow its dose schedule to lower the risk of stomach and gut reactions. It adds that if a dose is not tolerated, a clinician can consider waiting 4 weeks before the next step up.

The 2022 tolerability study found the same timing. In that study, most gut side effects came during or shortly after dose escalation. The Ozempic label says most reports of nausea, vomiting or diarrhea happened during dose escalation. To see the label steps laid out week by week, read our semaglutide dosage chart.

How Long Does Wegovy Diarrhea Last?

No source gives a set number of days. The STEP 1 trial says nausea and diarrhea were typically transient and subsided with time.

STEP 1 also says these effects were mostly mild to moderate. The 2022 tolerability study gives more detail. In that study, 99.5% of gut side effects on semaglutide were non-serious. The same study found 98.1% were mild to moderate.

If a page tells you diarrhea lasts an exact number of days, that number does not come from these trials.

Can Wegovy Cause Severe Diarrhea?

It can, but severe cases are much less common. The Wegovy label reports severe stomach and gut reactions in 4.1% of people on Wegovy injection, against 0.9% on placebo.

The Wegovy label notes that severe gut reactions have also been reported after approval with GLP-1 medicines as a group.

The label says Wegovy is not recommended for people with severe gastroparesis. That is a condition where the stomach empties very slowly.

Can Ozempic Cause Severe Diarrhea?

It can, but it is rare in the Ozempic label. The Ozempic label reports severe stomach and gut reactions in 0.4% to 0.8% of people, and in 0% on placebo.

Ozempic is semaglutide approved for type 2 diabetes. The Ozempic label says most reports of nausea, vomiting or diarrhea happened during dose escalation. It reports severe gut reactions by dose:

The Ozempic label reports severe stomach and gut reactions at these trial doses:

0.5 mg: 0.4% · 1 mg: 0.8% · placebo: 0%

Disclaimer: this is label information, not a recommendation. Dose decisions belong with a licensed clinician.

These Ozempic figures come from different trials than the Wegovy ones. The people and doses differ, so the two labels are not a direct head to head test.

Can Wegovy Diarrhea Cause Dehydration?

Yes. The Wegovy label says diarrhea, nausea and vomiting may cause a loss of fluids, which may cause kidney problems.

The Wegovy label tells patients it is important to drink fluids to help lower the chance of dehydration.

The Wegovy label reports acute kidney injury in people treated with semaglutide. Some cases needed dialysis. Most of these events happened in people whose gut reactions, such as nausea, vomiting or diarrhea, led to dehydration.

This is why ongoing diarrhea matters more than a single loose stool. The concern in the label is the fluid you lose.

Should I Stop Taking Ozempic If I Have Diarrhea?

The labels used here give no rule to stop on your own. The Wegovy label tells patients to contact their healthcare provider right away if diarrhea does not go away.

That label line covers nausea, vomiting or diarrhea that does not go away. Ozempic and Wegovy are both semaglutide.

Whether to pause, slow the dose steps or stop is a decision for a licensed clinician. This page only reports what the labels and trials say.

Why Are People Stopping Wegovy?

Stomach and gut side effects are one of the main reasons. In the Wegovy label's trials, the most common side effects that made people stop were nausea, vomiting and diarrhea.

The Wegovy label reports these rates of stopping:

  • Nausea: 1.8% on Wegovy, 0.2% on placebo
  • Vomiting: 1.2% on Wegovy, 0% on placebo
  • Diarrhea: 0.7% on Wegovy, 0.1% on placebo

The STEP 1 trial counted all gut events together. In STEP 1, 59 people on semaglutide (4.5%) stopped because of them. On placebo in STEP 1, 5 people (0.8%) did.

A 2026 real-world study looked at people outside a trial. In that study, 10.4% stopped semaglutide because of non-severe gut side effects. The same study found 5.7% stopped because of cost.

What Is Not Known About Wegovy Diarrhea?

Five common claims about Wegovy diarrhea have no support in the label or the trials. Here is what we looked for and could not find.

  • A day count. The label and trials say diarrhea was transient. They give no number of days.
  • A detox effect. No official or primary source on GLP-1 medicines says diarrhea flushes out toxins.
  • A one-month weight figure. None of the sources used here give one.
  • Constipation relief steps. None of the sources used here give them.
  • A rule to stop by yourself. The Wegovy label says to contact a healthcare provider if diarrhea does not go away. It does not set a stop rule.
