How Long Can You Stay on Zepbound? What the Label Says
How long can you stay on Zepbound? The FDA label sets no end date. It approves Zepbound to reduce excess body weight and maintain weight reduction long term. The longest trial found treated people for 176 weeks, which is more than three years.
The trials also show what happened when people stopped. In SURMOUNT-4, people switched to placebo after 36 weeks saw a 14.0% weight gain by week 88. This page sets out the label rules, the trial lengths and the stopping data.
Short answer
- Is there a time limit?
- No label or trial sets a maximum number of weeks or years. The label use is long-term weight maintenance.
- Longest trial data
- 176 weeks on treatment, then 17 weeks off, in SURMOUNT-1.
- What stopping did in a trial
- From week 36 to 88, placebo meant a 14.0% weight gain; staying on tirzepatide meant a further 5.5% loss.

What the Zepbound label says about treatment length
The label frames weight treatment as long term and gives no stopping point. Its weight use is to reduce excess body weight and maintain weight reduction long term. It covers adults with obesity, or adults with overweight and at least one weight-related condition.
The FDA label for Zepbound lists, for weight reduction and long-term maintenance:
Maintenance dose: 5 mg, 10 mg or 15 mg, injected under the skin once weekly
Disclaimer: this is label information, not a recommendation. Dose decisions belong with a licensed clinician.
The label says to weigh treatment response and tolerability when choosing the maintenance dose. If a dose is not tolerated, it says to consider a lower one. It does not give a review date or a length after which treatment ends.

How long on 2.5 mg Zepbound?
Four weeks, by the label. The FDA label for Zepbound lists:
Starting dose: 2.5 mg injected under the skin once weekly for 4 weeks
Then: 5 mg once weekly, with further steps of 2.5 mg after at least 4 weeks on the current dose
Disclaimer: this is label information, not a recommendation. Dose decisions belong with a licensed clinician.
The label says the 2.5 mg dose is for treatment initiation and is not approved as a maintenance dose. The full chart is in our tirzepatide dosage guide.

How long people stayed on tirzepatide in the trials
Trial lengths are the best guide to how long tirzepatide has been studied. In the trials cited here, treatment ran up to 176 weeks. The table below also lists the label's dosing periods, like the 4-week start at 2.5 mg, which are dosing steps and not trial lengths.
| Period | Length | Detail |
|---|---|---|
| Starting dose (label) | 4 weeks at 2.5 mg | For starting treatment only; not approved as a maintenance dose |
| Each dose step (label) | At least 4 weeks on the current dose | Steps of 2.5 mg up to a maintenance dose |
| Stomach side effects (SURMOUNT-1) | Mostly in the first 20 weeks | During the dose increases; mostly mild to moderate |
| Label withdrawal study (Study 4) | Up to 36 weeks, then up to 88 weeks | 783 people treated, 335 of them up to 88 weeks |
| SURMOUNT-4 switch point | Week 36 to week 88 | Some kept taking tirzepatide; others were switched to placebo |
| Longest trial treatment (SURMOUNT-1) | 176 weeks, then 17 weeks off | Adults with obesity and prediabetes |
The 176-week figure comes from SURMOUNT-1, in people with obesity and prediabetes. They took tirzepatide or placebo for 176 weeks. Then they were followed for 17 weeks off treatment. No trial found runs longer.

Do I have to stay on Zepbound long term?
No label makes it mandatory, but trials found weight returned for most people who stopped. The 2026 SURMOUNT-MAINTAIN trial looked at adults with obesity. It concluded that long-term treatment is often necessary to keep the weight off, along with the heart and metabolic benefits.
SURMOUNT-4 reached a similar view. Stopping tirzepatide led to substantial regain of lost weight. Staying on it kept the loss and added to it. Whether to continue is a decision for each person and their clinician, not one a trial can make.

What happened when people stopped tirzepatide
In the main stopping trial, most people who came off tirzepatide regained weight. SURMOUNT-4 treated everyone for 36 weeks, then switched some participants to placebo until week 88.
- Weight change from week 36 to 88: -5.5% on tirzepatide against +14.0% on placebo.
- Kept at least 80% of the earlier weight loss at week 88: 89.5% on tirzepatide against 16.6% on placebo.
- A 2026 follow-up analysis notes that most adults withdrawn after 36 weeks regained weight.
A 2025 meta-analysis of stopping GLP-1 drugs found that weight regain was proportional to the original weight loss. In that analysis, participants taking semaglutide or tirzepatide regained 9.69 kg.
The label gives the half-life of tirzepatide as about 5 days. That means the amount in the body falls by about half every 5 days after the last dose.

Lowering the dose instead of stopping
One 2026 trial tested a step down rather than a stop. The SURMOUNT-MAINTAIN authors wrote:
Reducing to 5 mg tirzepatide might be an alternative to discontinuation, although individual response might vary
Disclaimer: this is a trial finding, not a recommendation. Dose decisions belong with a licensed clinician.
The label makes a related point. If a maintenance dose is not tolerated, it says to consider a lower one. For weight change in the trials, see our tirzepatide before and after guide.

Side effects over three years of tirzepatide
In the 176-week trial, no new safety signals appeared. Apart from COVID-19, the most common side effects were stomach related. Most were mild to moderate, and most came during the dose increases in the first 20 weeks.
That means the trial saw no new kind of problem with longer use. It does not cover use beyond 176 weeks. For the stomach side effects themselves, see does Zepbound cause heartburn and tirzepatide diarrhea.

Questions about staying on Zepbound
How long can you stay on Zepbound?+
The Zepbound label sets no end date. It is approved to reduce excess body weight and maintain weight reduction long term. The longest trial data found ran 176 weeks of treatment, followed by 17 weeks off treatment.
How long on 2.5 mg Zepbound?+
The label lists 2.5 mg once weekly for 4 weeks as the starting dose, and says 2.5 mg is for starting treatment and is not approved as a maintenance dose. After 4 weeks the label moves the dose to 5 mg once weekly. This is label information, not a recommendation.
Do I have to stay on Zepbound long term?+
No label requires it, but the trials show weight tends to return after stopping. The 2026 SURMOUNT-MAINTAIN trial concluded that long-term treatment is often necessary to maintain weight reduction. Whether to continue is a decision for each person with a licensed clinician.
What happens if you stop Zepbound?+
In SURMOUNT-4, people switched to placebo after 36 weeks gained 14.0% from week 36 to week 88, while those who kept taking tirzepatide lost another 5.5%. The authors concluded that withdrawing tirzepatide led to substantial regain.
Can you take a lower dose of Zepbound instead of stopping?+
The label says a lower maintenance dose can be considered if a dose is not tolerated. The SURMOUNT-MAINTAIN trial said reducing to 5 mg might be an alternative to stopping, though individual response might vary. Dose decisions belong with a licensed clinician.
