FOXO4-DRI: Senolytic Peptide Research in Cells and Mice
FOXO4-DRI is a lab-made peptide, first described in a 2017 Cell paper, that breaks the link between the FOXO4 protein and p53 so senescent cells self-destruct. In fast aging and naturally aged mice it restored fitness, fur density and kidney function, but every study found is in cells, lab-grown tissue or mice.
Short answer
- What does FOXO4-DRI do?
- It frees p53 in senescent cells, which pushes those cells into apoptosis, a built-in cell death program.
- Does it work in people?
- Unknown. No human trial was found. The only human material tested is cartilage cells and keloid tissue in the lab.
- Any warning signs?
- Yes. A 2023 mouse study links clearing senescent lung blood vessel cells to worse blood flow in the lungs.

What Is the Difference Between FOXO4 and FOXO4-DRI?
FOXO4 is a protein your cells make, and FOXO4-DRI is a lab-made peptide built to block one of its jobs. They are not the same thing.
FOXO4 is a transcription factor, a protein that turns genes on and off. It is reported to be high in senescent cells. Senescent cells are aged cells that linger in tissue. Inside them, FOXO4 binds p53 in the nucleus and keeps the cell alive and in place.
FOXO4-DRI stands for forkhead box O transcription factor 4 D-Retro-Inverso. Researchers describe it as a senolytic peptide, meaning one aimed at clearing senescent cells.

How FOXO4-DRI Clears Senescent Cells
It works by pulling p53 away from FOXO4. Here is the chain of events reported in the research:
- In senescent cells, FOXO4 holds p53 in the nucleus, which stops the cell from dying.
- The 2017 team designed a FOXO4 peptide that disrupts the FOXO4 and p53 link.
- In senescent cells, this pushes p53 out of the nucleus and triggers the cell's own death program.
A 2025 structure study looked closer. It found that FOXO4-DRI binds a flexible region of p53, called p53TAD2, and the two form a briefly folded complex. Another 2025 paper notes that chemotherapy can make cancer cells senescent, and that these cells often resist cell death and help tumors come back. That is one reason researchers are studying FOXO4 and p53 blockers in cancer.

What the 2017 Mouse Study Found
This is the study that made FOXO4-DRI known. In mice, the peptide restored fitness, fur density and kidney function, in both a fast aging strain (XpdTTD/TTD) and naturally aged mice.
The same paper tested it against doxorubicin, a chemotherapy drug. Under conditions where it was well tolerated in the mice, the peptide neutralized the drug's toxic effects. That wording means the result holds for the conditions where the mice tolerated it well.
Every Tissue FOXO4-DRI Has Been Tested In
After 2017, researchers tried FOXO4-DRI on one tissue at a time. Most results are positive, one is a warning, and all are in mice or lab-grown human cells.
| Tissue | Model | What was reported | Year |
|---|---|---|---|
| Whole body, fast aging and naturally aged mice | Mice | Fitness, fur density and kidney function restored | 2017 |
| Testes | Naturally aged mice | Better testicular environment; age-related low testosterone secretion eased | 2020 |
| Sperm | Naturally aged mice | Increased sperm quality | 2024 |
| Aorta | Naturally aged and induced aging mice | Aortic aging suppressed; aortic function improved | 2025 |
| Cartilage cells | Human cells grown in a dish | More than half of the PDL9 cells removed; PDL3 cell numbers not significantly affected | 2021 |
| Keloid scar tissue | Lab-grown tissue and fibroblasts | Promoted cell death in pro-senescence models | 2025 |
| Lungs | Mice | Lung blood vessel lining cells lost; lung blood flow changed | 2023 |
The cartilage study needs a note. PDL9 and PDL3 are the paper's labels for how far the cells had been expanded in the dish, with PDL9 the more expanded. FOXO4-DRI removed more than half of the cells at PDL9 but did not significantly change the number of PDL3 cells. The authors added that whether it helps those cells form cartilage needs further investigation.
The testes result is mouse data, not a testosterone treatment. For what is known about testosterone more widely, see peptides and testosterone.

The Lung Warning From a 2023 Study
Clearing senescent cells is not always good, at least in mouse lungs. A 2023 study concluded that removing senescent lung blood vessel cells with senolytic treatments may worsen blood flow and pressure in the lungs.
In that study, changes in lung blood flow and loss of lung blood vessel lining cells occurred in mice given either ABT263 or FOXO4-DRI.
What Is FOXO4-DRI Used For in People?
It has no approved use in people. No human trial of FOXO4-DRI was found in PubMed, Europe PMC or ClinicalTrials.gov, and no FDA label or tested human dose was found.
So the honest summary is short: a targeted idea with strong mouse results, one mouse warning and no human data. For other compounds studied for aging, see peptides for anti-aging and the epitalon profile. For P021 and the p21 protein, two other names from aging research, see our p21 peptide guide.
