How long does it take for Wegovy to start working? The Wegovy label says the drug peaks in the blood 1 to 3 days after a shot, but weight change takes months. In the STEP 4 trial, people had lost 10.6% of body weight on average after 20 weeks. In the STEP 1 trial, the average change reached -14.9% by week 68. Under the label, the dose climbs for the first 16 weeks.

How long does it take for Wegovy to get into your system?
A few days for each shot, and about a month to build up. The Wegovy label says semaglutide reaches its highest level in the blood 1 to 3 days after a dose.
Each weekly shot adds to what is left from the last one. The Ozempic label, which uses the same drug, says a steady level is reached after 4 to 5 weeks of weekly shots. The drug also leaves slowly. The Wegovy label gives a half-life of about 1 week, and says semaglutide stays in the blood for about 5 to 7 weeks after the last dose.

Why does Wegovy start at such a low dose?
The slow start is on purpose. The label says the step-up is there to lower the risk of stomach and bowel side effects.
The FDA label for Wegovy injection lists, once weekly:
- Weeks 1 to 4: 0.25 mg
- Weeks 5 to 8: 0.5 mg
- Weeks 9 to 12: 1 mg
- Weeks 13 to 16: 1.7 mg
- Week 17 onward: maintenance dose

When does semaglutide start working on appetite?
No source we found tracks appetite day by day in the first week. One trial did measure how much people ate on weekly semaglutide.
In that trial, in people with obesity, total energy intake across free-choice meals over a day fell by 24%. In the same trial, body weight fell by 5.0 kg on average, mostly from body fat. So people on the drug ate less. The trial does not show how soon a person feels it.

What do Wegovy results look like after 3 months?
The closest trial point is week 20, a little under 5 months. In STEP 4, people took 16 weeks of dose step-up and 4 weeks at the maintenance dose.
After those 20 weeks, 803 people (89.0%) who had reached the maintenance dose went on to the main part of the trial. STEP 4 reported their average weight loss over the 20-week lead-in as 10.6%. No official source we found gives a 3-month or 1-month average. Before and after photos are not data, so this page does not use them.

How much weight is lost on Wegovy by week 68?
STEP 1 ran for 68 weeks. STEP 1 reported that by then, the average change in body weight was -14.9% with semaglutide and -2.4% with placebo.

How long does it take for the Wegovy pill to work?
The tablet has its own slower climb, in 30-day steps. Its maintenance dose starts on day 91.
The FDA label for Wegovy tablets lists, once daily:
- Days 1 to 30: 1.5 mg
- Days 31 to 60: 4 mg
- Days 61 to 90: 9 mg
- Day 91 onward: 25 mg

In the OASIS 4 trial of oral semaglutide, the average change by week 64 was -13.6%, against -2.2% on placebo. No earlier time point was found. A different semaglutide tablet, Rybelsus, also starts low: its label calls its 3 mg starting dose not effective for blood sugar control.

Do Wegovy side effects show up in the first weeks?
Mostly during the climb. The Ozempic label says most reports of nausea, vomiting or diarrhea happened during dose step-up.
